If you’d rather complete a paper form, you can download and print a consignment reorder form here and post it to us, email it to us or simply give it to your local bredent representative.

Please note: LOT numbers are required and can be found on the packet. All SKY implants come with labels inside the pack to help you keep track of your LOT numbers.
